Is it possible that Sherlock got a hold of that H.O.U.N.D. chemical and used it on John to create the illusion/nightmare that he died in the eyes of only John.
I can believe that all the surrounding people could have been staged there(most obviously/hopefully the bicycle guy), via homeless network. But I don’t see how Sherlock could fake a dead bleeding version of himself!
Therefore, I believe he got Molly to get the chemical and make it in a way it could be injected or something(not gas), to use on John if needed. Yes? No? Maybe so…… This speculation is gonna kill me O.O
